  • Existing Policy Documents 

When you buy a health insurance policy, your insurer provides you with a copy of your policy documents. These are crucial as they contain the policy number and other important information required during renewal. So, before you renew, make sure to keep the soft copy or hard copy ready. 

  • Health Declaration 

Health insurance companies need to know your health status to understand the level of risk they are taking on. Usually, if you are below 50, you don’t need to undergo any health checkups. However, if you develop any major complications during your policy period, you must disclose them at renewal. Otherwise, you may face difficulties when you claim. 

  • Renewal Form 

Many companies also require a renewal form to be submitted. Here, you have to provide your personal and policy information, such as your name, address, and policy number. You must provide correct information and update any details you want to change. This step is necessary if you choose the offline renewal process. 

What Documents Do You Need to Buy New Health Insurance Plans?

Knowing the documents required for renewal is necessary, but you should also learn which documents are needed to buy new health insurance plans. They are:

  • Age Proof 

To buy a new health insurance, you need to show proof of age, as it is the primary driver of your premium amount. You can use: 

  • 10th or 12th mark sheet
  • Birth certificate 
  • Voter ID card
  • Aadhaar Card
  • Driving license

You don’t need all of these to prove your age. Just keep one handy or scan a copy for storage on your phone. 

  • Address Proof 

Based on your location, your insurer decides the premium amount you pay. Therefore, having a valid address proof is necessary. 

  • Aadhaar Card
  • Electricity bill
  • Ration card
  • Voter ID Card
  • Passport 
  • Telephone bill 
  • Driving license

For easy access, keep your Aadhaar card or driver’s license handy so that these can qualify for both age and address proof. 

  • Identity Proof

Lastly, you need to provide proof of identity to purchase health insurance plans. You can use these documents for the same:

  • PAN Card

  • Passport 
  • Voter ID Card

Apart from these, your insurer may also want 2 passport-sized photographs at the time of buying. 

  • Medical Reports

What are the Documents Required for a Health Insurance Claim? 

Now, let’s move on to the documents you need to file a claim with your health insurer. 

  • For Cashless Claim 

  • Health Card: Your insurer will provide you with a health card containing your policy number and renewal date. You need to present it at the hospital. 
  • ID Proof: You also need a government-approved identity card, such as an Aadhaar or PAN card. 
  • FIR Copy: In case of an accident, you must file a First Information Report (FIR) at your nearest police station.  
  • Doctor’s Prescription: You also need a doctor’s prescription stating that the insured must be hospitalised. 
  • For Reimbursement Claim

  • Original Medical Bills: For reimbursement claims, you need to provide original receipts and hospital bills containing a breakdown of expenses. 
  • Claim Form: Next, you need a duly filled claim form. You can find it on your insurer’s website or app. 
  • Bank Details: You also need to provide a cancelled cheque or bank account details for NEFT transfers. 
  • ID Proof: Lastly, you must provide a valid ID proof.
